Checklists for the Application for Certification
The following is a list of checklists available to the laboratory to assist in completing the application package.
- Biosolids Preparation for Fecal Coliform Analysis (pdf) - This checklist provides guidance on applying for certification to perform fecal coliform analysis in biosolids (sludge) samples.
- EPA Method 1669 Checklist (pdf) - This checklist provides laboratories guidance on low-level metals sampling. This is currently only applicable to low-level mercury analysis.
- Inorganic Checklist - This checklist provides in-state laboratories a list of items required to complete the application package for inorganic parameters such as residual chlorine, pH, turbidity, Biochemical Oxygen Demand (BOD), etc.
- Organic Parameters Checklist (pdf) - This checklist provides in-state laboratories a list of items required to complete the application package for organic parameters such as EPA Methods 524.2, 624, 8260B, 8011, 608, etc.
- Out-of-State Checklist (pdf) - This checklist provides laboratories located outside of South Carolina with a list of items required to complete the application package. This checklist encompasses inorganics, organics, trace metals, and toxicity.
- PT Recertification Checklist (pdf) - This checklist is for laboratories that lost certification as a result of failing to meet the annual PT requirements.
- Residual Chlorine Checklist (pdf) - This checklist was designed specifically for drinking water systems ground water well systems.
- Trace Metals Parameters Checklist (pdf) - This checklist provides in-state laboratories a list of items required to complete the application package for trace metals parameters such as EPA Methods 200.9, 200.7, 6020, and SM3111B, SM3113B, etc.
- Toxicity Checklist (pdf) - This checklist provides in-state laboratories a list of items required to complete the application package for toxicity testing
Example Forms
Below is a list of forms developed by this Office. The forms are available to the laboratory in .pdf format. The laboratory can use these forms as they are or can modify them. The forms below provide the information reviewed by this Office. These forms may require additional information to make them complete and/or traceable based on how and where they are being used. They are provided as a service to the regulated community.
- Chlorite Analysis form (pdf) - This form is designed for laboratories performing chlorite and/or chlorine dioxide analyses.
- Dissolved Oxygen (DO) form (pdf) - This form is designed for laboratories performing dissolved oxygen analyses.
- Hydrogen-Ion Concentration (pH) form (pdf) - This form is designed for laboratories performing pH analyses.
- Initial Demonstration of Capability (IDOC) form (pdf) - This form is designed for laboratories performing organic analyses. The form includes all required information to make the necessary review possible. Please ensure that when applying for certification, the IDOC studies submitted include all of the information included on this form.
- Method Detection Limit (MDL) form (pdf) - This form is designed for laboratories performing organic analyses. The form includes all required information to make the necessary review possible. Please ensure that when applying for certification, the MDL studies submitted include all of the information included on this form.
- Residual Chlorine form (pdf) - This form is designed for laboratories performing total or free residual chlorine analyses.
